Mini Grant Report Form

As a past grantee, please submit a report about the grant you received and the impact it made.

The HTSDEF raises funds to enhance the school district’s existing curriculum, and is conscientious about how we spend these funds.
Your report should include the following:

  • How the funds benefitted or impacted the students or families of the district.
  • Was the funded project a success?
  • What was the impact?
  • Who did it benefit, and in what way?
  • Please explain, in observable and measurable terms, if the outcome was a success.
